What is a Blastocyst?

A blastocyst is a stage of early embryonic development that occurs around five to six days after fertilization in mammals, including humans. It is a critical milestone in the development of an embryo and represents a more advanced stage compared to earlier embryonic stages like the zygote and cleavage stages.

During the first few days after fertilization, the embryo undergoes multiple cell divisions, resulting in a cluster of cells. By approximately day three after fertilization, the embryo reaches the cleavage stage, consisting of about 6 to 8 cells. As the embryo continues to divide, the cells start to differentiate and specialize into specific cell types.

  1. Ovarian Stimulation and Egg Retrieval: The IVF process begins with ovarian stimulation using fertility medications to promote the growth and maturation of multiple eggs. Once the eggs reach the desired size, they are retrieved from the ovaries using a minor surgical procedure called transvaginal ultrasound-guided egg retrieval.

  2. Fertilization: The retrieved eggs are then fertilized in the laboratory with sperm from the male partner or a sperm donor. Fertilization can occur through conventional insemination (placing sperm directly with the eggs) or intracytoplasmic sperm injection (ICSI), where a single sperm is directly injected into each egg.

  3. Embryo Culture: After fertilization, the resulting embryos are placed in a specialized culture medium in an incubator. The embryos are monitored and allowed to grow and develop for several days under controlled conditions. On day three after fertilization, embryos are typically at the cleavage stage (around 6 to 8 cells). However, in blastocyst culture, embryos are cultured for an extended period, usually until day five or six, when they reach the blastocyst stage.

Blastocyst Grading: During the blastocyst culture, embryologists evaluate the embryos’ quality based on specific criteria, such as the degree of blastocyst expansion, the appearance of the inner cell mass, and the trophectoderm. Higher-quality blastocysts have a better chance of successful implantation.

  5. Selection of Embryos for Transfer: At the blastocyst stage, embryologists can better assess which embryos are more likely to result in a successful pregnancy. They select one or more high-quality blastocysts for transfer based on factors like the woman’s age, the number of embryos available, and previous IVF outcomes.

  6. Blastocyst Transfer: The selected blastocysts are gently transferred into the woman’s uterus using a thin catheter. The transfer is usually done on day five or six after fertilization. The goal is to synchronize the embryo’s development with the uterus’s receptive state, increasing the chances of successful implantation.

  7. Cryopreservation (Optional): If there are additional high-quality blastocysts that are not transferred immediately, they can be cryopreserved (frozen) for future use in frozen embryo transfer (FET) cycles.

  8. Pregnancy Test: About ten to fourteen days after the blastocyst transfer, a pregnancy test is conducted to determine if the procedure was successful.
